The 2026 Plan Highlights leave-behind booklet is a concise resource that provides an alternative to the traditional enrollment booklet. This compact guide includes key plan details, benefit highlights, and QR codes for easy access to tools like the UHC mobile app and Drug List, making it easier to support electronic enrollments with a more streamlined and efficient format.

The 2026 Plan Highlights is intended for use during electronic enrollments. If you choose to use it during a paper enrollment, you must also provide the following documents to consumers which can be downloaded from the Sales Material Portal starting October 1:

• Enrollment Checklist
• Summary of Benefits
• Medicare Star Ratings Document
