Great News! Beginning with 11/1/2021 effectives, Empire is increasing the incentive from $50 to $100 for brokers who take the extra time and effort to complete and submit Health Risk Assessments (HRA) for new members enrolling in a D-SNP plan.
In addition to the incentive increase, you will soon see a shorter more consolidated HRA that will be available for 11/01/2021 effective dates and the time to complete and submit the HRA has been extended to seven calendar days from the point of sale.

You have the option to either:
Reimbursement payments of $100 per HRA will be processed once the new D-SNP application is approved and the new member is active and will be included in your commission statements. HRA completions for current members moving to or switching between D-SNP plans are not eligible for payment. Reimbursement for HRA completion will be paid to the writing agent identified on the enrollment application within 60 days of the policy's effective date.
