D-SNP Benefits – Healthy Foods Card and $0 Prescription Copay

New for 2023 – Part D prescriptions and healthy foods are more affordable for Wellcare D-SNP* members! Members will pay $0 for covered prescription drugs and get help making healthy food choices with our Healthy Foods Card benefit. These along with supplemental benefits like dental, vision, hearing, OTC, transportation, and others help support the member's overall health.

Healthy Foods Card

The Healthy Foods card is EASY to use!

  • Members will automatically receive a prepaid debit card in the mail.
  • $25 or $50 allowance applied at the beginning of each month (amount varies by plan) (unused funds DO NOT carry over into the next month).
  • Purchase items In-store and On-line at participating retailers.
  • No Prior Authorization or Referral Needed on plans covering the benefit.
  • Covered items can include fresh, canned, and frozen food, prepared meals, and nutritional shakes (vary by vendor). 

$0 Prescription Drugs

Adhering to prescription regimens help keep members healthy!

  • Members pay $0 for covered prescription drugs, all year long in all benefit phases.
  • Generic and brand medications included. 

*Wellcare NJ Liberty plan and Ascension Complete D-SNP plans are excluded from this program.

Important Notes:

  • Covered items and retailers vary by card vendor.
  • Arkansas D-SNP Members (Serviced by Solutran) - Shop at Walmart, Walmart.com, Kroger, Dollar General, and other participating retailers for covered healthy food options. Members can access the Catalog and Store Locator at www.healthybenefitsplus.com.
  • D-SNP Members - Outside of Arkansas (Serviced by Incomm) – GA Foods, Mom’s Meals, Farmbox and participating retailers provide healthy foods. Members can access the Catalog and Store Locator at www.mybenefitscenter.com.    
  • Cards are not activated when mailed. Activation required once the card is received using the phone number on the card.
  • Healthy Foods Card amounts vary by plan. Check specific plan benefits before discussing with beneficiaries.
