The MyHealth Questionnaire (also known as the Health Risk Assessment or HRA survey) will no longer be accepted for additional administrative payment for 2025 enrollments. This change is in place for all applications with an enrollment effective date on or after January 1, 2025. This includes any 2025 enrollment applications that are submitted during the upcoming Annual Enrollment Period. This means the $75 additional payment for completed MyHealth Questionnaires will no longer be made moving forward.
If you have a completed MyHealth Questionnaire with a 2024 enrollment application submitted through Nov. 30, 2024, the survey will still be accepted, and the additional $75 payment will be made.
