UPMC for Life Complete Care (HMO D-SNP) has just earned 5 out of 5 stars!
UPMC is proud to announce that the UPMC for Life Complete Care (HMO D-SNP) plan in Western and Central PA (H4279) has earned an excellent 5 out of 5 stars from Medicare for 2024! This award places the Special Needs Plan among the highest-rated Medicare plans in the nation. UPMC for Life is incredibly proud to say that their HMO, PPO, and D-SNP plans in Western and Central PA have all earned an excellent 5 out of 5 stars!
What does this mean? Those eligible for our D-SNP plans get a 5-star SEP now too!
Those eligible for D-SNP can enroll in the Western and Central PA D-SNP plans by using the 5-star SEP election. This includes plan benefit packages H4279-001 and H4279-004. This does NOT include H7123-001 (in Southeastern PA). Eligible Medicare beneficiaries can disenroll from their current plan and enroll in a Medicare Advantage plan offered that currently has a 5-star overall rating. The eligible individual can only use the SEP one time from now through Nov. 30, 2024.
