UPMC For Life AOR Process

Below are important change to the agent of record (AOR) policy for UPMC for Life Medicare plans.  The agent of record is entitled to receive certain information and assist members in managing their coverage with UPMC for Life. Beginning October 15th 2024, existing UPMC for Life members wishing to change their AOR may do so by completing an AOR change letter.

Here are some important details on how the change letter process works:

• The letter must be signed by the member or a representative who is authorized under state law to sign legal documents on their behalf.
• Each letter is only valid for a single member. Additional change requests must be completed through a separate letter.
• Agent of Record change requests received between October 15th 2024 through December 31st 2024 will be effective January 1st 2025.  Requests on or after January 1st 2025 will be effective the first of the following month.
• Completed letters should be faxed to UPMC for Life at 412-454-7766.
• Agent of Record change letters may not be attached via Cavulus.
• The Agent of Record Change letter is limited to member-initiated change requests. Sale of book or transfers between agents may not be effectuated through the Agent of Record Change letter process.
• UPMC for Life members originally enrolled by internal UPMC for Life employed agents are not eligible for Agent of Record change to an external broker. Only limited exceptions to this policy may apply based on member circumstance.
• UPMC Health Plan reserves the right to approve or reject any Agent of Record request in its sole discretion.
