Consumers eligible for a UnitedHealthcare Chronic Special Needs Plan (C-SNP) may be able to enroll through a Special Election Period (SEP)1, which helps them find a plan that suits their needs. Some UnitedHealthcare C-SNPs are tailored for individuals with a qualifying chronic condition and full Medicaid, although Medicaid is not a requirement for enrollment. The plan benefits are designed to focus on medical needs, and the member's Medicaid and LIS cover the full cost of prescription copays.
Why C-SNPs are beneficial for members
Use the new C-SNP meeting flyer and postcard in the UHC Agent Toolkit to promote informational meetings. Search keywords "C-SNP Meeting Flyer" or "C-SNP Postcard."
UnitedHealthcare is committed to providing their members with comprehensive support that goes beyond just health insurance. Here are a few important items you can use to help them make the most of their plan.

With certain plans, UnitedHealthcare works closely with providers and/or care organizations to take a team approach to members' health care to help manage their overall well-being and help prevent health problems before they start.
The new "Care and Support" page is live on UHC.com, and an entire section of the page provides more information on what it means to be in a plan that includes a team approach to health care.
