The Clear Spring Health HRA Form is Now Online

Great news! Clear Spring Health has digitized their HRA form AND have an incentive for YOU!

Earn $80 for each eligible HRA for beneficiary's enrolling in one of their MAPD plans.

Earn $100 for each eligible HRA for beneficiary's enrolling in one of their DSNP plans.

Health Risk Assess (HRAs) are critical to the well-being of Medicare beneficiaries because they help companies like Clear Spring Health effectively manage members’ care.

You may also locate our HRA by visiting the CSH website, and selecting For Brokers, and Broker Overview.

To use the new online HRA form, you will need to create an account using your NPN and email address on file with Clear Spring Health. To complete your registration, you will be prompted to confirm your email address by clicking on the link that will be emailed to you.

Once you’re all set, you may begin submitting HRAs, and will receive a confirmation number at the end of each for future reference. It’s really that easy!
