Submitting Clover Health Applications

Submitting enrollment applications with Clover Health is simple.

See below for tips & tricks from the carrier!

IMPORTANT:

• All broker enrollment applications must be submitted from the Plan Documents and Enrollments page on the Clover Health website!
• ~~During this process, you will submit your NPN to be recorded as the AOR (Agent of Record).
• ~~If you do not see the option to input your NPN on the enrollment application, you are on the incorrect application!

Ensure you input your NPN on the application you are submitting!

• If you do not see this option on the enrollment application, you are on the incorrect application.
• Submit applications through the Plans Documents and Enrollments page on the Clover website

If you submitted an application without recording your NPN and would like to be the Agent of Record - you may contact Clover Health's Broker Experience team to correct it : 1-855-979-2236 or brokers@cloverhealth.com

You MUST have the following items to be added as the AOR:

• A PDF copy of the Scope of Appointment
• A copy of the enrollment receipt (a screenshot of the enrollment confirmation)
• Agent NPN
• Member Name
• Member Date of Birth
