Dental, Vision, & Hearing application process update

Proof of prior dental coverage required for waiting period waiver.

Starting November 21, Cigna Healthcare^SM Supplemental Benefits Flexible Choice Dental, Vision & Hearing (DVH) applicants are required to provide proof of prior dental coverage documentation to waive the Class III waiting period.

If a customer answers "yes" to the prior coverage question when applying for a DVH policy, one of the following must be submitted:

• Prior policy ID card
• Summary of Dental Coverage including coverage dates
• Explanation of Benefits (EOB) including date of service
• Copy or photo of recent bill or invoice

This documentation can be provided either at:

• the point of sale
  • via the Document Upload section (if applying via Express App)
  • as a paper copy (if applying via paper/U.S. Mail) or
• while the application is pending in Book of Business on CignaForBrokers (go to Policy Details > Policy Information> Document Upload).
