

UnitedHealthcare and Endeavor Health in Illinois have reached agreement on a new multi-year contract. This agreement ensures people enrolled in UnitedHealthcare Medicare Advantage plans as well as UnitedHealthcare's Dual Special Needs Plan (D-SNP) have network access to Endeavor’s providers and the following hospitals in the Chicago metropolitan area:
Ensure members are aware that Endeavor Health will continue to remain in UnitedHealthcare's Medicare Advantage network with no change. Encourage members who have questions about in-network health care providers to call the customer service number on their UnitedHealthcare UCard® or use the provider directory on myuhcmedicare.com.
Agents can refer to the provider directory on Jarvis > Provider Search to confirm a provider’s status and view a list of accepted plans.
