Molina Broker-Submitted HRA Forms Deadline EXTENDED!
Extended deadline: Broker-submitted HRA forms
The deadline for brokers' submission of completed HRAs for Medicare members has been extended to February 28, 2022. This extension applies to members having effective dates of Jan. 1, 2022 and Feb. 1, 2022.
- The HRA form is available here: https://molina.callidusinsurance.net/ICM/
In compliance with HIPAA laws and Molina Healthcare policy, the HRA form must be emailed securely to: MolinaBrokerHRA@molinahealthcare.com.
In the subject field, enter "SEND SECURE Broker Sales HRA."
For encryption, please follow the method compatible with your Windows, iOS or Android device.
