Highmark: 2025 MA Enrollment Kit Updates

Highmark would like to inform you that two (2) CMS-mandated updates have been made to the 2025 Enrollment Applications within the following 2025 Medicare Advantage (MA) Enrollment Kits:

• Blue Rx (PA & WV) – English, Spanish
• Central Pennsylvania (CPA) – English, Spanish
• Delaware (DE)
• Northeastern New York (NENY)
• Northeastern Pennsylvania (NEPA) – English, Spanish
• Southeastern Pennsylvania (SEPA) – English, Spanish
• West Virginia (WV)
• Western New York (WNY)
• Western Pennsylvania (WPA)

What You Need to Know:

Highmark has made the following CMS required updates to the MA Enrollment Applications:

• Section 2: The voluntary Sexual Orientation and Gender Identity questions have been removed.
• Attestation of Eligibility for an Enrollment Period (“Other Reason”): Beginning April 1st, any individuals who wish to use the Disaster/Emergency SEP must call 1-800-MEDICARE to make an election. The FEMA language previously used in this section of the application has been removed, and we have added a note describing the above scenario.

What You Need to Do:

1. Dispose of any affected existing inventory you may have, and reorder new kits as needed.
2. Updated 2025 MA Enrollment Kits will be available for ordering by 3/14.
3. Kits with the updated 2025 Enrollment Applications will begin shipping by 3/26 to guarantee delivery by 4/1. The updated applications can also be found:
   1. In the digital Enrollment Kits in DCatalog
   2. Online via Plan Documents
