Excellus is very excited about their produce enhancements for 2023 and think you’ll be happy with the changes as well!

Below is the Supply Request form for 2023 MA PD Sales Materials. For printed, pre-AEP orders, please submit this kit request form via email to the ExcellusBSD@excellus.com email. Order forms must be received by September 10 at the latest, to receive supplies by 9/30/22.

Please remember, Star Ratings will be updated and must be replaced in the sales kits in mid-October, so please keep that in mind as you contemplate quantities.

All orders placed after the new Star Ratings are available will be updated prior to distributing.

Also, please note that the Medicare Supplement kits are for 2022. New applications may be required after December.
