Here's How It Works:

• Submit each completed HRA electronically or by completing a fillable HRA PDF.
• Incentive payment per HRA will be processed once the new application is approved and the new member is active.
• HRA completions for current members switching plans are not eligible for payment.
• Incentive payments will be included in commission statements.
