Which Plans Offer the Grocery Benefit?

• NJ: H5141-001, 004, 007, 054 / H8010-002, 003
• AL, AZ, GA, MS, PA, SC, TN, TX : All‍‍

Which Plans do NOT the Grocery Benefit?

• NJ: H5141-032, 042, 055 (055 has neither grocery nor OTC)

Ways to Purchase Grocery?

• In-store
• Pick-up & Delivery* - At checkout, enter card details to use allowance on approved items and allowable shipping fees. (Not all shipping costs are covered by the health plan.)
• Order online at www.HealthyBenefitsPlus.com/CloverHealthOTC to have approved foods shipped directly to the home.
• Prepared Meal Delivery* - Order ready-to-heat, prepared meals from Mom’s Meals and have them delivered to the home.

How To Use Card In Store:‍

• Generally, cashier (or member if self checking) can scan the barcode to process the payment at checkout
• If scanning doesn’t work, follow the grid to process the deduction of the benefit allowance from the total amount of transaction.
