CMS prohibits the use of the Medicare name, logo, and Medicare card image in a misleading way. All materials that include an image of the Medicare card must obtain prior authorization from CMS. Follow the process below:

The Centers for Medicare & Medicaid Services (CMS) allows for a Special Enrollment Period (SEP) when a Medicare beneficiary with other creditable drug coverage is moving from a Medicare Advantage plan with drug coverage (MAPD) or a prescription drug plan (PDP) to a Medicare Advantage (MA) only plan or Original Medicare. Use code 'CDC' on the enrollment form to enroll customers using this SEP. You must ensure all customers meet the eligible criteria for this SEP.
