

Good news for the start of AEP! The broker incentive for completion of the Health Risk Assessment (HRA) for new D-SNP Plans is increasing from $50 to $100! In addition to the incentive increase, the HRA form was shortened. This means it takes even less time to complete, and the time allowed to submit the HRA has been extended to seven calendar days from the point of sale.*
These changes begin with new D-SNP plans effective 11/1/2021. Below you'll find branded versions of Anthem's HRA “Quick Start Guide” and a marketing flyer, along with a copy of the fillable HRA PDF.

*NOTE: SunFire does give agents the ability to submit the completed HRA forms. Additionally, SunFire has provided a process document (linked below) outlining the process for those downline brokers using SunFire.
